How to Make an Appointment

• By Appointment Only.  Create your telehealth or in-person appointment using the following steps (Thursday is reserved for telehealth and in-person visits on Friday):

  • STEP 1.  Click "Request an appointment" button to receive initial enrollment form (basic demographics, past medical history, medications, etc.).

  • STEP 2.  Within 24 hours you will receive a "Welcome email" giving "OK to make your appointment" and directions for choosing the appropriate appointment day and type based on your medical situation.  (Pulmonary patients are seen only in-person.) (CHECK SPAM if you do not receive your Welcome email w/in 24hrs.)

  • STEP 3.  Click "Make an appointment" button in that email and schedule your appointment.

  • STEP 4.  For telehealth visits.  You will receive a link for your visit at the time of your appointment.  See details in your "Welcome email."
