Appointments

How do I make an appointment?
Step 1.  "Request an appointment". Our appointment process begins by clicking one of website's "Request an appointment" tabs.  This leads you to our enrollment process where patients submit information such as demographics, medical and medication histories, credit card information, Practice policies etc.  (Thursdays are reserved for telehealth visits and Fridays for in-person--Pulmonary patients must be seen in-person.)

Step 2.  Make an appointment.  Once submitted, we review your situation and within 24 hours invite you to make your appointment. With the invitation you will receive a "Welcome email" with directions to choose the appropriate day and type of appointment for your individual medical complaint.  (CHECK SPAM if your Welcome email is not received.)
Step 3.  Reminders.  After you have made your appointment and acknowledged our Practice Policies you will receive periodic appointment reminders.  Please carefully review our no-show / late cancellation policy for all the reasons described in those policies.  
Step 4.  Day of appointment.  If telehealth, you will receive the link shortly before your appointment.  Please arrive 15 minutes prior to in-person appointments so we can ensure that we start on time.  
Step 5.  Check out.  Check out is completed by the doctor after your appointment--on the same day.  Your credit card is charged and a receipt is sent to you.  A detailed summary of services can be provided if requested to allow you to submit a claim to your insurance company.
