Privacy Policy

The Perry Center for Pediatric and Adult Sleep Care (the Practice, Us, We) operates the care and benefit of our Potential Patients, (Potential Patient, Patient, or You). The Practice agrees to provide You with the service and protect the Us and You by adopting the privacy policies described below.​ Patients in the care of the Practice are provided a complete compilation of all Privacy policies and procedures prior to arriving for an appointment. Potential Patients are provided only those policies associated with Communications to allow for multimodal communications prior to becoming a patient.​
Communication Policy​

1. General. The Practice endeavors to provide Potential Patients with the convenience of a wide variety of electronic communication options. Although we are careful to comply with patient confidentiality requirements and make every attempt to protect Your privacy, communications by email, facsimile, video chat, cell phone, texting, and other electronic means, can never be absolutely guaranteed secure or confidential methods of communications. Communications initiated by Potential Patients (by calling the Practice) may receive a reply by one or more electronic means. By reaching out to the Practice (by phone) you allow a response by any confidential means chosen by the Practice and you expressly waive any guarantee of absolute confidentiality with respect to their use.​

2. Unsolicited Communications. The Practice DOES NOT send unsolicited communications.The Practice used multiple electronic means to RESPOND to inquiries made by Potential Patients.​

3. Email Usage. By providing an email address in correspondence with the Practice, the Potential Patient authorizes the Practice and its staff to communicate with him/her in like manner regarding the Potential Patient "protected health information" (PHI).The Potential Patient further understands that:
A. Email message are not necessarily secure methods of sending or receiving PHI, and there is always a possibility that a third party may gain access.
B. Email messaging are not appropriate means of communication in an emergency, for dealing with time-sensitive issues, or for disclosing sensitive information. Therefore, in an emergency or a situation that could reasonably be expected to develop into an emergency, the Potential Patient agrees to call 911 or go to the nearest emergency care facility and follow the directions of personnel.​

3. Texting. By texting to correspond with the Practice, the Potential Patient authorizes the Practice and its staff to communicate with him/her in like manner regarding the Potential Patient "protected health information." Texting often allows for more efficient and timely responses to questions asked by Potential Patients.
A. The Potential Patient understands that texting allows for reasonable confidentiality but is not a guarantee of absolute securing.
B. Texting are not appropriate means of communication in an emergency, for dealing with time-sensitive issues, or for disclosing sensitive information. Therefore, in an emergency or a situation that could reasonably into develop into an emergency, the Potential Patient agrees to call 911 or go to the nearest emergency care facility and follow the directions of personnel.
C. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
